Purpose
This privacy statement (the “Statement”) is provided by Salt Lake City School District (“District” or “SLCSD”) in compliance with Utah Code § 63D-2-103.
SLCSD is committed to protecting your privacy and this Statement explains how the District collects, uses, and protects your information when you visit a District-sponsored webpage..
Scope
This statement applies to all District and District-authorized charter school websites.
Who We Are and How to Reach Us
SLCSD, as a public school district, operates this governmental website, which is hosted by Finalsite. Additionally, the District uses other websites that are managed internally and by other third parties.

Definition of Personally Identifiable Information
Personally identifiable information (“PII”) refers to any information that can be used to identify an individual. Examples of PII include, but are not limited to:
· Name
· Physical address
· Phone number
· Email address
· Driver’s license or other Identification (“ID”) number
Collection and Use of Information
Below is an explanation of how your information may be collected and used when you visit a District website:
· Web Forms: web forms can be used to collect name, email, phone, address, qualifications, languages spoken, availability, and student registration information for the purpose of communicating with District staff, volunteering, and/or enrolling your students in school.
· Usage Data: all District websites collect Internet Protocol (“IP”) address, browser type, domain names, referring and exit pages, URLs, pages viewed, order of pages viewed, the date and time the website was accessed, and other diagnostic data.
· Device Information: device information such a “desktop”, “mobile”, operating system, and MAC address may be collected.
· Location Information: generic physical and geographic locations are collected based upon the IP address used to connect to District websites.
· Cookies: the District website and third parties acting on our behalf may use cookies and other tracking technologies for the purpose of analyzing website traffic, identifying site visitation, and storing preference information.
· Finalsite Privacy Policy: review Finalsite’s privacy policy for additional information that may be collected by the District’s website hosting service.
· Student information: for information regarding the collection of student information, please visit the District’s Student Data Privacy webpage.
Data Security and Integrity
SLCSD places the highest priority on data security. The District has taken adequate measures to ensure the confidentiality, integrity, and security of all information entered into District websites. All District internal systems are placed behind several layers of security to protect data from loss, misuse, or unauthorized access. Authorized third parties contractually agree to protect District information using the same level of security. Additionally, District staff are required to complete information technology security training annually.
Please note that while the District has placed a high level of security and redundancy on District systems and processes, the District cannot guarantee against the occurrence of system/hardware failure, human error, or unauthorized intrusion.
Third Party Websites
This and other District websites may link to third-party, non-District websites. Many of these websites may not be subject to the same state and federal laws that apply to the District’s websites. When visiting third party websites, be sure to review the privacy policies of each third-party website to gain an understanding of how they will collect and use your information.
